I am 61 with severe what my doctor calls "marionette lines." What is the procedure for correcting these?
The marionette lines are the lines that run from the corner of the mouth to the chin and are actually the extension of the jowls. The marionette lines can be treated with either a non-invasive treatment or a surgical procedure.
The non-invasive treatment of marionette lines involves the use of facial fillers. I particularly like the hyaluronic fillers, such as Perlane, for this area. The skin is numbed with a topical anesthetic cream for about 10 minutes. Perlane now comes pre-mixed with a local anesthetic for a pain-free injection. The procedure takes about 10 minutes and the patient can return to work the same day. The majority of patients that undergo correction of marionette lines with fillers are very satisfied.
The surgical correction of marionette lines involves a facelift procedure. The facelift procedure can be done with local anesthesia with oral sedation, IV sedation, or general anesthesia. The procedure takes about 3 hours and the recovery is about 2 weeks. The procedure involves a tightening of the facial muscles and their repositioning to achieve a fresher look. The procedure works well to improve the sagging neck, jowls, marionette lines, and the midface. Most patients that undergo the facelift procedure are very happy with the results.

What filler should I use for restoring volume to hollow cheeks ...Radiesse vs Juvederm?

I have used both Juvederm and Radiesse to restore volume to the cheeks. Both work well, given that an adequate volume of filler is used. That being said, I personally favor Radiesse for cheek augmentation. It lasts longer and gives fantastic results. In fact, this is my preferred use for Radiesse.
Radiesse can be mixed with local anesthesia and injected in a layered fashion to give a more youthful appearance. Results are immediate and there should be minimal bruising.

I feel like I have very droopy cheeks at the corners of my mouth. Do any fillers work to provide a sort of cheek lift in this area?
This is a common reason that people use fillers around the mouth. Patients usually ask me to improve the look and contour of this area. The lip corners are an area where several lines and folds unite. From above, the nasolabial fold ends on the corner of the lip. If this is prominent, it can make this area seem heavy and can hide the corner of the lower lip. In addition, the heaviness can translate to a downturned lower lip, making the patient seem sad. I treat this area with a combination of fillers and Botox. I start with filling the nasolabial fold to decrease its depth. I then use filler to fill and elevate the lower lip corner. The last step is to place a small droplet of Botox on the lower lip muscle to relax it. The relaxation of this muscle helps to elevate the lower lip just a little bit. The result is a beautiful lower lip and a happy patient!

Siempre he tenido labios muy finos y me gustaría hacer algo para hacerlos más carnosos. ¿Cuáles son los mejores rellenos para agregar volumen a los labios y cuánto duran los resultados?
Mi relleno favorito para labios es el Restylane. Creo que funciona muy bien y da un aspecto muy natural cuando se coloca en el lugar y la cantidad adecuados. Añado una franja muy fina alrededor de los labios inferior y superior. Después coloco una cantidad moderada de relleno dentro de los labios. Esto produce un aumento moderado de la eversión o carnosidad del labio. Cuando se hace de una forma moderada, los labios se pueden rellenar con un resultado muy natural. El procedimiento se realiza en el consultorio y normalmente lleva unos 10 minutos. Se aplica crema anestésica local en los labios y luego se inyecta el Restylane. Una innovación reciente consiste en agregar anestésico local de lidocaína al Restylane, lo cual ha logrado reducir la incomodidad asociada a las inyecciones de un volumen importante.
